Presenting a case of GB perforation after PCNL (Percutaneous Cystolithotomy) performed for Right renal stone. We performed a laparoscopic surgery & the patient recovered well.

- Free fluid in abdomen along with absent of free gas under diaphragm was pointing towards GB perforation as a possible diagnosis.
- Never discharge a patient who complaints of post procedure new onset abdominal pain.
- Biliary peritonitis managed laparoscopically have rapid recovery.
